PROCUREMENT OF GOODS REQUIREMENTS CHECKLIST
Section 23.6:
ELIGIBILITY Check for the Procurement of Goods.
The determination of eligibility shall
be based on the submission of the following documents to
the BAC , utilizing the forms prepared by the BAC and using
the criteria stated in Section 23.11 of this IRR-A: |
1. CLASS “A” DOCUMENTS
[1.1] Legal Documents
a) DTI Business Name Registration or SEC
Registration Certificate, whichever may be appropriate under
existing laws of the Philippines - (Photocopy Only)
b) Valid and current Mayor’s Permit/
Municipal License (2006) – (Photocopy Only)
c) Taxpayer’s Identification Number
– (Photocopy Only)
d) Sworn Statement that prospective bidder
is not “blacklisted” or barred from bidding
by the Government, or any of its agencies, offices, corporations
or LGU’s, including non-inclusion in the Consolidated
Blacklisting Reports issued by the GPPB, once released in
accordance w/ the guidelines to be issued by the GPB as
provided in Sec. 69.4 of this IRR-A (Follow GSC-BAC Format)
e) Other appropriate licenses as may be
required by the procuring entity concerned (Valid License
to Operate (LTO) for suppliers of drugs/medicines &
medical supplies) |
[1.2] TECHNICAL DOCUMENTS
f) Statements of the prospective bidder
of all its ongoing and completed government and private
contracts within the relevant period, where applicable,
including contracts awarded but not yet started, if any.
The statement shall state for each CONTRACT whether said
contracts is: (Follow GSC- BAC Format)
f.1 Ongoing, Completed, or Awarded but
not yet started; within the relevant period, where applicable.
The statement shall include, for each contract, the following;
i. Name of Contract
ii. Date of Contract
iii. Kinds of goods sold
iv. Amount of contract & value of
outstanding contracts (Refer to Sec.23.11.1 item 2)
v. Date of delivery
vi. End User’s acceptance, if completed
and
vii. Specification whether prospective
bidder is a manufacturer, supplier or distributor
f.2 Similar or not similar in nature and
complexity to the contract to be bid.

[1.3] FINANCIAL DOCUMENTS
i) The prospective bidder’s audited
financial statements, stamped “received” by
the BIR or its duly accredited and authorized institutions,
for the immediately preceding calendar year, showing, among
others, the prospective bidder’s total and current
assets and liabilities; – (Photocopy Only) and
j) Prospective bidder ‘s computation
for it’s Net Financial Contracting Capacity (NFCC)
which shall be in accordance with section 23.11 of this
IRR-A; or a commitment from a licensed bank to extend to
it a credit line if awarded the contract to be bid, OR a
cash deposit certificate, in an amount not lower than that
set by the procuring entity in the Bidding Documents, which
shall be at least equal to ten percent (10%) of the approved
budget for the contract to be bid. (Refer to Sec. 23.11.1
Item 3)

2. CLASS “B” DOCUMENTS
a) Valid joint venture agreement, in case
of a joint venture. Each member of the joint venture shall
submit the required eligibility documents;
b) Letter authorizing the BAC or its duly
authorized representative/s to verify any or all of the
documents submitted for the eligibility check (Follow GSC-BAC
Format)

The prospective bidder shall certify under
oath that each documents submitted in satisfaction of the
eligibility requirements is an authentic and original copy,
or a true and faithful reproduction or copy of the original,
complete and that all statements and information provided
therein are true and correct. (Refer to Sec.23.8) |
Section 25.3:
The first envelope (TECHNICAL PROPOSAL) shall contain the
following technical information/documents, at the least: |
| 1. The Bid Security as to form, amount
and validity period: (Refer to Sec. 27.2)
2. Authority of the signatory;
3. Production/Delivery schedule;
4. Manpower requirements;
5. After-sales service/parts, if applicable;
6. Technical Specifications; (CPR for drugs and medicines,
and for medical supplies (if needed), brochures for medical
supplies/ equipment)
7. Bank Commitment to extend to the bidder a credit line
if awarded the contract to be bid, OR a cash deposit certificate,
in an amount not lower than that set by the procuring entity
in the Bidding Documents, which shall be at least equal
to ten percent (10%) of the approved budget for the contract
to be bid; Provided, however, that if the bidder previously
submitted this document as an eligibility requirement, the
said previously submitted document shall suffice;
8. Certificate from the bidder under oath of its compliance
with existing labor laws and standards, in the case of the
procurement of services;
9. A sworn affidavit of compliance with the Disclosure provision
under Section 47 of the Act in relation to other provisions
of RA 3019.
10. Other documents/materials as stated in the Instruction
to Bidders
11. Sworn Statement/Affidavit attesting to the responsibility
of bidder. (Refer to Sec. 17.7, Follow GSC - BAC Format)

Section 25.4:
The second envelope (FINANCIAL PROPOSAL) shall contain the
following financial information/documents, at the least; |
1. Bid prices in the Bill of Quantities
in the prescribed Bid forms; and
2. The recurring and maintenance costs, if applicable. |
